1. No person to be insured has:
   • a medical condition other than allowed medical conditions
   • in the last 12 months
     • suffered from; or
     • received treatment or advice for; or
     • been prescribed any drug or medicine for;
     any medical condition, other than allowed medical conditions;
   • in the last 6 months received treatment as an in-patient in a hospital or nursing home;
   • within the last 5 years been diagnosed with or treated for any form of cancer.
   
   
2. All persons to be insured are resident in the UK;
    
3. No person to be insured is
   • on a waiting list for treatment in a hospital or nursing home;
   • waiting for investigation or the results of any investigation, medical treatment or surgical procedure;
   • choosing not to take prescribed medication, or the correct dose of prescribed medicine.
   • travelling against the advice of a medically qualified doctor;
   
   
4. All persons to be insured are under 65 years of age.
    
5. I am a Santander customer.
    
6. I understand that this policy has certain exclusions, limitations and conditions which I accept.
